The HART Hub Dufferin is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use challenges. Role Overview
Family Transition Place (FTP) is seeking a dynamic and compassionate Addictions Counsellor to be an integral part of the HART Hub Dufferin collaborating with a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals. The Addictions Counsellor will conduct assessments, brief interventions, provide case management supports and facilitate groups.
This position will receive direction from the HART Hub Manager for day-to-day operations and is accountable to the FTP Program Supervisor. The Addictions Counsellor will work with both internal and external partners and will interact at all levels within the organization.
Responsibilities will include:
- Conduct comprehensive standardized addictions assessments including developing Crisis Plans for clients identified with substance use challenges and provide counselling to those with complex addictions.
- Provide support to individuals with mental health challenges who may also have identified substance use concerns.
- Offer motivational interviews by using the stages of change and a harm reduction philosophy.
- Assist with activities of daily living, supportive services, conflict resolution, and crisis avoidance.
- Consult with other team members to lead the development of educational programs focused within addictions which may include liaising with other community addictions services.
- Committing to FTP’s core values of Compassion, Integrity, Equity and Social Responsibility.
- Adhering to FTP and HART Hub policies and procedures and compliance with regulatory requirements such as OHSA, AODA, etc.
- Working with individuals in crisis situations.
- Perform additional duties as required/delegated by Management
Qualifications:
- Post-secondary education in Social Work, Health Services, Psychology or equivalent.
- Regulated health professional is an asset.
- Minimum 2 years experience in the mental health and addictions sectors with a focus on assessment, brief interventions and short term supports.
- Demonstrated understanding and a commitment to harm reduction principles and practices.
- Strong written and oral communication skills.
Employment Requirements:
- Vulnerable Sector Police Clearance Certificate and maintaining of clearance requirements during employment.
- Valid driver’s license and insurance.
- CPR and First Aid Certification
Working Conditions and Physical Requirements:
- 35 hours per week, including day, afternoon and weekend hours to be established.
- Able to work well with both internal and external clients.
- Travel by vehicle to community sites for client support as required.
Compensation: $28.60 to $31.77 per hour + allowance in lieu of benefits
