Role Overview

Family Transition Place (FTP) is seeking a dynamic and compassionate Housing Support Worker to be an integral part of the HART Hub Dufferin collaborating with a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals. The Housing Support Worker will provide Housing First focused support to increase housing stability and prevent eviction. The Housing Support Worker will provide housing case management supports, engage with landlords, identify housing opportunities, and support clients to secure and maintain safe housing.

This position will receive direction from the HART Hub Manager for day-to-day operations and is accountable to the FTP Program Supervisor. The Housing Support Worker will work with both internal and external partners and will interact at all levels within the organization.

Responsibilities will include:

  • Ensure clients are fully informed of Housing Support Workers role, Coordinated Access mission and philosophy.
  • To assist with goal planning, a full SPDAT assessment to be completed according to the determined timeline. This will aid in identifying areas so case management may be prioritized.
  • Meet with clients on a weekly basis for an hour at minimum.
  • Providing ongoing individual and group intervention and support to program participants.
  • Providing support, options, referral and liaison with community resources and agencies as required.
  • Connecting with land-people, matching people to housing, conducting housing advocacy and teaching life skills.
  • Providing referrals for clients as required.
  • Committing to FTP’s core values of Compassion, Integrity, Equity and Social Responsibility.
  • Adhering to FTP and HART Hub policies and procedures and compliance with regulatory requirements such as OHSA, AODA, etc.
  • Perform additional duties as required/delegated by Management

Qualifications:

  • Post-secondary education in Social Work, Social Services, Health Services or equivalent.
  • Minimum 2 years supporting the homeless population.
  • Strong knowledge of Housing First Principles.
  • Strong written and oral communication skills.

Employment Requirements:

  • Vulnerable Sector Police Clearance Certificate and maintaining of clearance requirements during employment.
  • Valid driver’s license and insurance.
  • CPR and First Aid Certification

Working Conditions and Physical Requirements:

  • 35 hours per week, including day, afternoon and weekend hours to be established.
  • Able to work well with both internal and external clients.
  • Travel by vehicle to community sites for client support as required.

Compensation:          $28.60 to $31.77 per hour + allowance in lieu of benefits
