through holistic, trauma-informed, in-home support. As a Caseworker, you will play a key role in providing
comprehensive, trauma-informed case management supports to families enrolled in the Chrysalis program.
This involves engaging all members of the family unit (caregivers, children, and fathers/partners) in the
journey toward healing, safety, and resilience. You will collaborate with families to assess needs, create
safety and transition plans, set goals, and facilitate access to resources that promote long-term stability and
healthy relationships.
This is a hybrid, dynamic and mobile position, requiring flexibility across home visits, community-based
support, remote work, and in-office responsibilities, to meet the unique and evolving needs of each family.

Case Management
• Provide individualized, holistic support to all family members through in-home visits and community based services.
• Support families in navigating legal, housing, financial, child/youth, and other necessary systems.
• Facilitate short- and long-term goal setting, safety planning, and transitional support.
Service Coordination and Advocacy
• Conduct intake meetings with all clients, assess needs, develop plans of care, and create safety and
transitional plans for all members where necessary.
• Maintain accurate, confidential case files and record all data into the program database.
• Make referrals to external agencies and ensure successful follow-through via warm transfers and
follow-ups.
• Build relationships and collaborate with community partners to ensure families are supported
through well rounded care and connected to appropriate services.
Program Development and Team Engagement
• Actively contribute to team meetings, training, and organizational development initiatives.
• Contribute to the implementation and refinement of the Chrysalis Project model.
• Uphold trauma-informed, anti-oppressive, anti-racist principles in all interactions with clients, staff,
and community partners.

• Excellent interpersonal, communication, and crisis intervention skills.
• Flexible, organized, with the ability to manage multiple priorities in a fast-paced environment.
• Empathetic, collaborative and community centred.
• Proficiency in Microsoft Office and case management software.
• Valid CPR/First Aid certification and Vulnerable Sector Criminal Record Check.
